Services and ProgramsIn every county, one agency is funded to maintain up-to-date information, to conduct in-home needs assessments, provide information about services to meet those needs, to develop appropriate care plans and arrange for services. Outreach efforts are targeted to those older adults who are unaware of available services or benefits and who are difficult to contact. Outreach specialists advise older people (or their representatives) of available benefits and services, encourage them to participate and assist them in receiving benefits under insurance and other public programs. Service LocatorThe Service Locator is designed to help older persons, family members and caregivers locate local support resources. When you use the Service Locator, you will obtain individual agency contact information.
