The following is an updated comprehensive list of reports and forms that should be sent to me at the Kankakee Office.

Questions regarding Elder Abuse cases should be directed to Eric Weakly at the West Chicago office.  Questions regarding funding/payment issues and services monitoring should be directed to me.

Questions regarding waivers for FSS, FCS or Elder Abuse services may be directed to Eric, and if he is not available ask for me, Walter or Maryanne.

NAPIS NUTR.DATA ENTERED (for prev month): Due the15th each month

IIIE CCC NAPIS DATA ENTERED (for prev month): Due the 28th of each month

TITLE V FISCAL REPORTS: Due the 5th each month

ANE INTAKES: Due the 5th each month (by 3:00 p.m.)

ANE RECONCILIATIONS: Due the 20th each month

ANE REQUEST: PAYMENT of MONEY MNGMNT: Due the 20th each month

FLEXIBLE SERVICES SPENDING: Due the 10th each month

QUARTERLY:

GRANTEE FINANCIAL STATUS REPORTS: Jan 10, Apr 6, July 10, Oct 6 & Close-Out due: Nov 30
