Lombard, IL – AgeGuide, the Area Agency on Aging, is pleased to announce new and expanded partnerships for a variety of services for older adults in Lake County. New partnerships were formed with Antioch Area Healthcare Accessibility Alliance (AAHAA) and Waukegan Township to enhance the Aging and Disability Resource Network (ADRN) in Lake County. This collaboration aims to improve access to resources and services for older adults and people with disabilities in our community.
ADRN services will be provided by one of three organizations based on the township in which the older adult lives.
- Antioch Area Healthcare Accessibility Alliance: Antioch, Avon, Grant and Lake Villa
- AgeGuide: Ela, Cuba, Fremont, Moraine, Vernon, Wauconda, and West Deerfield
- Waukegan Township: Benton, Libertyville, Newport, Shields, Warren, Waukegan, and Zion
The Aging and Disability Resource Network (ADRN) provides vital support, information and assistance, benefits access and Medicare counseling to help people navigate services and supports that help people age well in the community. With the addition of these new partners, we are well-positioned to seamlessly continue delivering these services and ensuring quality resources for those in need.
AgeGuide has also expanded its existing partnership with two organizations. ElderCARE, which currently provides friendly visits and phone calls, will now also provide transportation services for older adults. White Crane Wellness, which currently provides health promotion services, will also serve as the new provider of the Community Connection Collaboratives program.
“These new and expanded partnerships reflect AgeGuide’s commitment to creating a more inclusive and supportive environment for older adults in Lake County,” said Marla Fronczak, Chief Executive Officer of AgeGuide. “Together with these partners, we will work tirelessly to ensure that older adults and people with disabilities have the resources and support they deserve.”
These provider changes will be effective starting October 1, 2024. For more information about older adult services available in Lake County, visit www.AgeGuide.org.
