New Older American Act Services Providers

AgeGuide is pleased to announce the addition of new partner agencies and expanded partnerships for a variety of Older Americans Act services for older adults across the region. These partnerships aim to improve access to information and assistance; transportation; Adult Protective Services; and Community Connection Collaboratives (Health, Education & Recreation), ensuring older adults receive the support they need to live fulfilling lives.

Grundy County: 

• Catholic Charities, Diocese of Joliet will now provide Aging & Disability Resource Network services
• Grundy Transit, in conjunction Catholic Charities, Diocese of Joliet, will now provide transportation options

Kendall County: 

• Oswegoland Senior & Community Center will now provide the Community Connection Collaborative program

Lake County: 

• Aging & Disability Resource Network services will be provided by one of three organizations based on the township in which the older adult lives
  • Antioch Area Healthcare Accessibility Alliance: Antioch, Avon, Grant and Lake Villa
  • AgeGuide: Ela, Cuba, Fremont, Moraine, Vernon, Wauconda, and West Deerfield
  • Waukegan Township: Benton, Libertyville, Newport, Shields, Warren, Waukegan, and Zion
• ElderCARE will now provide transportation services
• White Crane Wellness will now provide the Community Connection Collaboratives program

Will County:

• County of Will, in conjunction with Catholic Charities, Diocese of Joliet will be providing door-to-door transportation services
