Lombard, IL – The Board of Directors of AgeGuide Northeastern Illinois elected Katie Monahan Brooks to the Board as a Lake County representative. She will serve a three-year term on the Board of Directors.
Ms. Monahan Brooks is the Manager of Business Development for Transitions Hospice. She provides healthcare continuing education to both the professional and consumer, builds partner relationships and guides prospective patients through the palliative care and hospice process.
“AgeGuide is pleased to welcome Katie to the Board of Directors,” said Marla Fronczak, Chief Executive Officer for AgeGuide. “Katie has a heart for service to older adults. We believe her professional and volunteer experience will be influential to AgeGuide as she serves her term on the Board of Directors.”
Katie serves in several volunteer leadership positions for organizations serving older adults. She is the State Board Vice President of Continuity of Care, Vice President of Continuity of Care – Lake McHenry Chapter; President of the Senior Service Coalition of Lake County, and the founding member of the Coalition for Aging Resources & Education. She is the past Corporate Sponsorship Chair for the Lake County Walk to End Alzheimer’s and past Vice President of the Lake County Behavioral Health Alliance. Katie has also served on several professional advisory committees for both hospice and not-for-profit home health agencies.
Katie earned her Bachelor of Music in Music Therapy from Alverno College in Milwaukee and has 15+ years of experience in the healthcare industry. A commitment to respect and professional values of servant- leadership brought her to her current role with Transitions Hospice in 2019.

AgeGuide Northeastern Illinois, an Area Agency on Aging, is a nonprofit organization responsible for developing and coordinating a network of services for older adults throughout an eight-county area.
