AgeGuide Northeastern Illinois is one of over 600 such agencies throughout the United States, created by Congress to implement the Older Americans Act and coordinate a wide variety of services for persons 60 and older. AgeGuide awards funds to local organizations in each county within our designated area to provide these important services and monitors the organizations to ensure accountability and quality.

We strive to make aging easier through the development and promotion of a network of services for older persons designed to optimize the quality of their lives.

  • Information & Assistance

    Specially trained professionals are available to answer questions and connect you with local services and resources, such as:

    • Home delivered meals or community dining programs
    • Benefits programs that help pay for prescription drugs, health care, rent, utilities, and other needs
    • Support for yourself while taking care of an aging friend or relative
    • Rides to the doctor, store and for other errands
    • Assistance in the home such as light housework, personal care and meal preparation
    • Ways to stay engaged and active in your community

  • Options Counseling

    Empowers individuals to make informed decisions about long term services and supports by providing objective information, advice, and counseling. Assistance is provided in accessing a range of public and private programs. Options Counseling is available to adults of all ages, as well as concerned others, making long term support decisions regardless of income or financial assets.

    Friendly Visiting provides regular visits by volunteers that are focused on developing a friendship and engaging in enjoyable and purposeful activities together.

  • Means & Nutrition Assistance

    Home Delivered Meals provide older adults with a daily hot meal, well-being check and a few moments of friendly conversation. Eligible clients receive meals mid-day, during scheduled delivery times, up to five days a week. Meals are approved by Registered Dietitians and provide 1/3 of the daily nutrition needs of the average older adult. In some areas, optional meals may be available for individuals needing pureed foods or vegetarian diets.
    A suggested donation is requested and appreciated for all meals, however, it is not required. There is no income or asset requirement for home delivered meals. No one is denied a meal if unable or unwilling to contribute. 
    Home Delivered Meals Eligibility:

    • Care Coordinator completes in-home assessment and referral for home delivered meals
    • Age 60+ and live in designated service area
    • Frail/home bound individuals
    • Unable to shop/prepare/obtain meals
    • Unable to attend community dining meals
    • Not receiving meals from another source
    • No other adult available/willing to prepare meals
    • Contact Care Coordination Unit (CCU) in your area for more information

  • Family Caregiver Program

    You may be a daughter, a son or a spouse, a sister or brother, a niece or nephew—or simply a good friend. You do what you need to do to help an older family member or friend in need. You are a family caregiver, and you are not alone!

    The Family Caregiver Support Program provides valuable resources for those who are giving unpaid care to adults age 60+ and for those with Alzheimer’s disease or a related disorder. The program includes:

    • Caregiver Resource Centers provides informaiton, resources and support for those giving unpaid care to adults age 60+ and those with Alzheimer’s disease or a related disorder, or for relatives age 55+ raising children under 18 or giving unpaid care to adults with disabilities. Services include respite services and supplemental funds to help pay for short term services, adaptive devices and supplies not covered by insurance.
    • Respite Services provide alternatives to take a break from caring of your loved one in the home or at another location.
    • Gap-Filling Services offers options to obtain support in unforeseen circumstances covering a variety of items to assist you.
    • Legal Services to help you take care of your loved ones.
    • Caregiver Counseling Centers provide resources to improve and protect the well-being of those giving unpaid care to adults age 60+ and those with Alzheimer’s disease or a related disorder, and for relatives age 55+ raising children under 18 or giving unpaid care to adults with disabilities. These programs help coach/counsel you through troubling times, such as how to bring the family together or to help the older adult accept needed services.
    • Training and education provides ways you can take care of yourself and to learn “how-to” care giving skills to aid your loved one.
    • Support groups provide a place to share experiences and understand that you are not alone.

    Caregiver Support Programs are available in the eight counties AgeGuide serves: DuPage, Grundy, Kane, Kankakee, Kendall, Lake, McHenry and Will.   • Adult Protective Services

    The Adult Protective Services Program responds to and investigates reports of abuse, neglect and financial exploitation of persons age 60+ and adults with disabilities aged 18-59 who live in the community.  Abuse is a term referring to any negligent act by a caregiver or any other person that causes harm or a serious risk of harm to a vulnerable older adult.

  • Nursing Home Advocates

    Long-term care ombudsmen are advocates for residents of nursing homes, board and care homes and assisted living facilities. They are trained to resolve problems and can assist with complaints of residents living in long-term care facilities. However, unless given the ombudsman permission to share concerns, these matters are kept confidential. Every state is required to have an Ombudsman Program that addresses complaints and advocates for improvements in the long-term care system.

  • Fraud Assistance

    The Senior Medicare Patrol (SMP) Program empowers consumers to prevent healthcare fraud. Program staff and volunteers help people with Medicare and Medicaid to:

    1. PROTECT themselves from Medicare & Medicaid fraud, waste, and abuse.
    2. DETECT Medicare & Medicaid fraud, waste, and abuse.
    3. REPORT Medicare & Medicaid fraud, waste, and abuse.

  • Legal Assistance

    Legal services include arranging for and providing assistance in resolving civil legal matters, protecting legal rights, providing legal advice, community legal education and conducting research concerning legal rights and responsibilities by an attorney-at-law or a person under the supervision of an attorney. Clients shall be 60 years and older who have a problem that requires assistance from an attorney or a paralegal. Efforts will be extended to those who are homebound and those residents who are in institutions. Case priorities shall be set to target basic human needs to include but not limited to health care, shelter, income maintenance and safety.

  • Transportation

    Transporting older persons to and from community facilities and resources for purposes of acquiring/receiving services, participate in activities or attending events in order to reduce isolation and promote successful independent living. Service may be provided through projects specially designed for older persons, use of public transportation systems, or volunteer program transportation.

  • Veterans Independence Program & Veterans Resources

    The Veterans Independence Program (VIP) is a Veteran’s directed home and community-based service program that serves eligible veterans of any age who are at risk of nursing home placement and their family caregivers. VIP allows Veterans to receive home and community-based services that enable them to avoid institutionalization and continue to live their homes and communities for as long as possible.

  • Health Promotion Classes

    A Matter of Balance

    This program teaches older adults how to reduce fall risks and eases their fear of falling, improves falls self-management, increases self-confidence, and promotes physical activity. It also helps eliminate the isolation and depression fear of falling causes.

    Bingocize

    A fun and engaging way tolearn,socialize,and exercise while playing the game of bingo. This program combines some falls prevention or nutrition information while strengthening muscle groups that are needed toimprove balance.

    Fit & Strong

    This class is a multiple component exercise program designed to help seniors with stiffness and pain to be more active and confident in their daily lives and allows them to live a more active lifestyle. The class provides physical activity training and health education for older adults with lower extremity osteoarthritis.

    Healthy Ideas

    Designed to detect and manage depressive symptoms among at-risk older adults and their caregivers. It is a focused intervention to support better management of depressive symptoms and increase engagement in meaningful activities.

    Tai Chi for Arthritis

    The focus is on improving movement, balance, strength, flexibility, immunity and relaxation while decreasing pain and falls. Tai Chi is one of the most recommended activities by many health-related organizations such as the Mayo Clinic, Harvard Health, Parkinson’s Foundation, and many more in the management of chronic pain, while maintaining flexibility.

  • Community Care Program

  • Employment Assistance

    The Senior Community Service Employment Program is an employment training program for individual’s ages 55 years and better. The program provides participants 20 hours per week of on-the-job training opportunities, at local nonprofits or government agencies, which assist in the development of new and marketable job skills to help you re-enter the workforce. All program participants receive a subsidized hourly wage for training to help offset costs associated with travel to and from their training sites.
